NPI Registry Corrections and Deletions
The NPI Registry (National Provider Identifier) contains over 3.5 million registrants of health care providers who have registered to receive Medicare payments. Finalized in 2008, the NPI numbering replaced the previous UPIN, OSCAR, PIN and NSC systems of identifying health care providers and suppliers. NPI is a standard, unique identifier mandated by the The Health Insurance Portability and Accountability Act (HIPAA). All health care providers are eligible for NPIs. Application forms are available from CMS (Center for Medicare and Medicaid Services), part of HHS (U.S. Department of Health and Human Services).
- NPI registrants SHOULD NOT submit their personal info such as cellphone number or home address to the NPI Registry. Registrations are public information and will be published in multiple locations.

The NPI Registry and the downloadable files will contain data from the NPPES as reported to NPPES by you, or by someone acting on your behalf, or by an organization provider's Authorized Official. If the downloadable file or the NPI Registry reflects information that is incorrect, health care providers should correct that information. At any time, providers, or someone acting on their behalf, may edit their records by going to https://nppes.cms.hhs.gov, or by obtaining a paper NPI Application/Update Form (CMS-10114) from the NPI Enumerator or from the CMS forms page (http://www.cms.gov/Medicare/CMS-Forms/CMS-Forms) and mailing the completed, signed form to the NPI Enumerator. Providers who need assistance in editing their records should contact the NPI Enumerator by phone at 1-800-465-3203, by email customerservice@npienumerator.com, or by letter: NPI Enumerator, P.O. Box 6059, Fargo, ND 58108-6059.
The information disclosed on the NPI Registry and in the downloadable files are FOIA-disclosable and are required to be disclosed under the FOIA and the eFOIA amendments to the FOIA. There is no way to ‘opt out’ or ‘suppress’ the NPPES record data for health care providers with active NPIs.
Here are the steps to correct or remove an NPI Registry listing:
STEP # 1 -- NOTHING CHANGES UNTIL YOU CORRECT THE NPI REGISTRY LISTING
Last NPI update: February. Next update: mid-March 2022.
1. If the listing has an NPI number, it is an NPI Registry listing.
Follow the above links or call the above telephone number. Make the correction with HHS.
If you are a third-party such as a former employer, or a wrong phone number: Call the above number and have the NPI number and provider name available when you call. Explain the situation, we have found them to be very helpful.
2. We will pick up the corrected NPI database on our regular monthly schedule. HHS makes the updated database available to us once per month. Our websites will have the corrected information displayed in 2-6 weeks.
3. Correcting the NPI Registry will correct listings on other websites that publish NPI information, not just our websites.
4. After the info has been corrected on our websites, to correct your listing in the Google index, go to Google's
remove information page. Follow instructions.
